1. Take the Tiered Fidelity Inventory (TFI) at least once this year online at pbisapps.org by the date you submit your online Recognition application. No minimum score (fidelity) requirement on the TFI for Recognition this year.
2. Submit an online Recognition application: https://bit.ly/RecSY2021 (PLEASE NOTE: If you have issues with your organization blocking this Google Form, please email eval@midwestpbis.org ) 3. Levels of implementation do not exist this year (Bronze, Silver, Gold, Platinum) 4. Badges now exist for items corresponding with the TFI at Tier 1, Tier 2, & Tier 3 5. Submit at least one piece of evidence for the specific Badge when prompted in your online Recognition Application. (All evidence for each Badge must be in a single document. The document may have multiple pages). |
